Clubs wishing to affiliate should fill in the bottom half of this form with their club name, together with the name, address and telephone number of their SpoCo contact. Then return it to Paul Warrener, at the address below, along with a cheque for £12.50 made payable to "Lakes and Lancashire SpoCo". Clubs requiring a receipt please include a stamped addressed envelope.

The main responsibility of the SpoCo contact is to ensure that your club members are fully aware of the Lakes & Lancs SpoCo and are given every opportunity to read the mailshots and league tables that will be sent to the contact throughout the duration of the competition (including the enclosed literature).

If you wish to nominate someone else as SpoCo contact, please feel free to do so. That person must sign the declaration below.
